Caravan Health Announces Second Annual Accountable Care Symposium

Austin, Texas, July 12, 2017 (GLOBE NEWSWIRE) -- Caravan Health is excited to announce its second annual Accountable Care Symposium, the premier learning and networking event for current and future participants in Advanced Payment Models. The landmark event returns to Phoenix, Arizona on December 6 and 7 at the Pointe Hilton Squaw Peak Resort, where hundreds of healthcare professionals will come together to facilitate high-level learning and thoughtful discussion about the advancement of value-based payment systems.   

The symposium will feature industry thought leaders as they shed light on the key-issues effecting the value-based care landscape. Attendees will participate in breakout sessions during the two-day event, where field experts will share their experiences and strategies for effective population health management.

The first day of the symposium will be dedicated to policy and physician engagement before breaking into specialized tracks that will address the real-world concerns of new and experienced Accountable Care Organizations (ACOs), and Transforming Clinical Practice Initiative (TCPI) and Comprehensive Primary Care Plus (CPC+) participants. The second day will highlight how practices can drive their Advancing Care Information (ACI) scores and cover Behavioral Health Integration (BHI), including practical content about how to address the challenges of opioid addiction. Taken together, the content will enable practices to implement workflows that will achieve the highest scores regardless of which program they joined or how far along they are in the transformation process.

The meeting will convene independent practice associations, small practices, rural health systems, community hospitals, FQHCs and multi-specialty groups; together with hundreds of current ACO participants including primary care physicians, community hospital CEOs, nurse care coordinators and executive leaders. Many newly accepted ACO participants will attend, joined by physicians and hospital leaders who are preparing to enter into Advanced Payment Models in 2018.
